- A valid mobile number and email address are requested in the online process.

What happens next
Submission is not the final step.
- The application is routed to the relevant upazila or urban social-services office based on the current address.
- The applicant is expected to attend the medical assessment at the time provided by the responsible office.
- The official instructions say to bring the printed copy generated after online submission to that medical assessment.
- The ID card is collected from the relevant social-services office after notification.
